A project affidavit is a signed declaration used in Lean Six Sigma certification to attest that a candidate completed a qualifying improvement project and to verify specified facts about that work.
For an external DMAIC assignment, the affidavit connects the candidate’s submission to someone with direct knowledge of the project. In an invoice-processing project, that person might be the host’s process owner or project sponsor, subject to the certification provider’s rules. The provider determines the required form and who is eligible to sign it. Agreement about verification belongs at the start of the assignment, before either party commits time to collecting evidence.
The declaration needs support from the project record. If an invoice project reports reduced rework, the supporting material should establish the baseline and follow-up measurement periods. It should also explain how rework was counted. Released staff time and cash savings require separate treatment: fewer processing hours do not establish a reduction in expenditure. A signatory needs enough visibility into the records to support the statements they are asked to verify.
The project charter establishes scope and authority before DMAIC work proceeds. The affidavit supports verification after completion, while the control plan assigns responsibility for continued performance. An affidavit does not replace the analysis files or evidence of approved changes. Where invoice records contain confidential information, the host and candidate should agree what supporting material may accompany the certification submission.
